Health Service Requirements for All Full-Time Students
All full-time students admitted to Lincoln College are required to complete health records. All records and documents are required to be complete and on file in the Health Service office prior to or on the check-in date of the entering semester. There will be a charge for late records.
All students must complete the following forms: Health History, Immunization, Tuberculosis Risk Questionnaire, and Student/Parent/Guardian Information. Proof of immunity can be provided by submitting one of the following: 1) Immunization form, completed and signed by a physician or public health official; 2) Copy of high school, health department, or physician’s office immunization record; 3) Copy of military records.
For students born after January 1, 1957, immunizations required are:
- Proof of tetanus/diphtheria booster, given within ten years of attendance at Lincoln College.
- Proof of two measles (MMR dates acceptable), both after one year of age and after December 31, 1968 and at least one month apart.
- Proof of at least one (preferably two) mumps immunization(s) (MMR dates acceptable), after one year of age and after December 31, 1968 and at least one month apart.
- Proof of at least one (preferably two) rubella immunizations (s) (MMR dates acceptable), after one year of age and after December 31, 1968 and at least one month apart.
Although not required, it is strongly recommended providing proof of immunity to polio and immunizations against Hepatitis B (series of three injections), meningitis (meningococcal) bacteria (one injection), Hepatitis A (one or two injections), chickenpox (if the student has never had chickenpox –one/two injections), and HPV (for females, a series of three injections).
Note: A physical examination is only required for those students participating in competitive sports, cheerleading or dance (poms). Physical examinations are valid for one calendar year.
Important: A student will not be allowed to participate without a physical on file with Health Services.
Information and instructions for meeting the stated requirements are provided to all incoming students. The Director of Health Services recommends submission of all required information as soon as possible and no later than the check-in date. Students not in compliance with the requirements will be charged a late fee. The student could be dismissed mid-term if not in compliance.
