Steps to Apply for Accommodations

E-mail – ncods@lincolncollege.edu - Register with ODS and submit current (within 5 years) documentation of your disability.
- Ask your diagnosing professional to complete the ODS Diagnosing Professional
form. Acceptable documentation will include:
- Professional diagnostic statement of the disability
- Current functional impact of the disability
- Treatments, medication, accommodations currently in use
- Expected progression of the disability over time
Note: A high school IEP or SOP, while helpful in determining effective accommodations, does not in itself constitute sufficient documentation. - Complete registration forms and sign a student responsibilities contract, all of which you can submit during the intake interview.
- Meet with ODS and request accommodations.
- ODS will issue a letter outlining approved accommodations which students should give to each instructor.
