Eligibility
Students requesting support services are required to submit documentation to verify eligibility under the Americans with Disabilities Act of 1990. The documentation must include medical or psychological information from a certified professional. In addition to providing documentation of a disability, students will be asked to complete an intake form. Once the student has been admitted, appropriate documentation and the intake form should be submitted to ODS prior to arrival. Eligibility must be established before services can be provided.
How To Register With ODS
- Contact ODS by phone or e-mail to request an intake packet.
- Submit the completed intake form (pdf 89KB) with documentation of a disability.
- Once documentation has been reviewed, the student will be notified of the status of his or her file.
- Once approved for services, the student schedules an appointment with the Director of ODS to complete the intake.
- During the intake appointment, appropriate accommodations and ODS policies and procedures will be discussed.
ODS reserves the right to determine eligibility for services based on the quality of the submitted documentation. All documentation is confidential and will be shared only with those who have a legitimate need to know.
